Types of evidence we can accept

When submitting your grant application to Family Fund, certain documents will help us when being able to assess your eligibility.

The type of document that is helpful for us to see are those which provide a lot of information around the additional support that your child or young person needs.

If dated within the last 12 months, we can accept the following documents:

  • A full copy of a report detailing the education support provided to help with your child or young persons special educational needs, such as a Statement of Special Educational Needs or an Individual Education Plan (IEP)
  • A supporting letter from a Teacher, SEN staff member, Key worker or Portage team
  • A therapeutical assessment report from a SALT, Neurologist, Audiologist, Clinical Psychologist or Paediatrician
  • A supporting letter from a Doctor, Health Care worker or Therapist

As part of your application, we will ask you to provide a document that names your child or young person. This is to ensure the person you are applying for lives with you. This could be a copy of an award notice for Disability Living Allowance (DLA) , Child Disabled Payment (CDP) , Personal Independence Payment (PIP) or Adult Disability Payment (ADP)

However, we can not accept these as evidence of your child or young person’s eligibility, nor can we accept appointment letters or health and educational reports that are older than 12 months.

Are we eligible?

Grants are available for families raising a child or young person with a long-term disability, disabling condition or life-limiting illness, and living on a low income. If your child needs a high level of support with things like personal care, education, communication, medical treatment, physical adaptations, you may be eligible to apply to us.
