In order to be compliant with Data Protection legislation you must read the following documents ‘GDPR Agreement for referrers’ and ‘Hospital Referrers Terms and Conditions and Data Protection Statement’:
When you have read these documents, if you want to proceed you must complete the following GDPR Agreement click submit within the next 28 days. If you do not wish to proceed you do not have to complete the GDPR Agreement. We will take non completion within the 28 days as confirmation of withdrawal of your request for registration.
Once we have received your agreement that you have read and understood the documents we will confirm your registration and supply you with the rest of the documents you will need to start making Hospital Applications.
